Project Description

Hill Country Community Clinic (“Hill Country”) is a Federally Qualified Health Center (“FQHC”) providing outpatient medical, dental, mental health, intensive case management and wellness services to residents of Shasta County, California. The project is new construction of a 40,000 sq. ft. facility that provided comprehensive medical and mental health services, substance abuse treatment, and recovery activities for patients with complex health and social needs and housing for 16 homeless transitional-aged youth. Hill Country Community Clinic offers integrated medical, dental, chiropractic, and mental health services to everyone, regardless of their ability to pay. With $11 million in NMTC sub-allocation and $10 million in loans, including investment from NFF’s AIM Healthy Fund and participation from Dignity Health, Hill Country Community Clinic is another step closer to the construction of a new 35,000 sq. ft. health center that will serve over 9,000 patients annually once fully operational. The project, Center of Hope, also includes 16 units of supportive housing for young adults transitioning out of homelessness.
