Project Description

Capital Impact Partners and Chase provided NMTC financing to Urban Health Plan (UHP), via its support corporation, The Foundation for Urban Health and Well Being, Inc. to expand their existing federally qualified health center (FQHC) located at 37-102 108th Street Corona, NY in Queens.

The project also includes the outfitting for the integration of UHP’s existing Logistics Center located at 343-347 Soundview Avenue in the Bronx, NY to provide medical supplies, inventory management systems, and storage to support over 80,000 unique patients throughout the UHP system.

In addition to providing standard medical services, UHP also improves the health of its community by removing barriers to access, addressing the social determinants of health, and developing targeted programming to address specific chronic illnesses. For example, UHP formed a coalition of community based organizations which coordinates on how to improve social determinants. In 2009, UHP founded the Dr. Richard Izquierdo Health & Science Charter School and is an institutional partner of the charter school, the first in New York City to offer a career and technical education program to prepare students for jobs in the health care sector.
